EHCP SMART Target Bank - Free SEN Resource

Created By: undefined

What is the EHCP SMART Target Bank?

The EHCP SMART Target Bank is a comprehensive professional resource designed to support the writing of high-quality, legally compliant SMART targets for Education, Health and Care Plans (EHCPs) and SEN Support Plans. Fully aligned to the SEND Code of Practice 2015 and the Children and Families Act 2014, it provides ready-to-use SMART targets across the four primary areas of need: Autism Spectrum Condition (Communication & Interaction), ADHD (Cognition & Learning / Communication & Interaction), Dyslexia and Specific Learning Difficulties (Cognition & Learning), and Social, Emotional and Mental Health (SEMH). Each target is written in professional SENCO language, includes clear success criteria, and specifies a recommended review timescale. The resource also contains a SMART Target Writing Planner, a Target Review Record, a Pupil Voice Sheet, a quick-reference guide to key SEND Code of Practice statutory sections, a Graduated Approach summary, and a SEND terminology glossary - making it a fully self-contained planning and review toolkit. The document spans 26 pages.

Who is this for?

This resource is written specifically for SENCOs (Special Educational Needs Co-Ordinator), class teachers, and SEND practitioners working in schools and educational settings across England. It is equally relevant for those writing new EHCPs, preparing for annual reviews, or developing SEN Support Plans under the Graduated Approach (Assess, Plan, Do, Review). It will also be of value to local authority SEND teams, educational psychologists, and any professional involved in co-producing person-centered SEND provision. The included Pupil Voice Sheet makes it accessible as a tool to involve children and young people directly in their own target-setting process.

How should it be used?

This resource is designed to be used as a practical reference at every stage of the EHCP and SEN Support planning cycle. Practitioners should begin by identifying the child's primary area of need, then select the most appropriate pre-written SMART targets from the relevant section, personalising each one by replacing the [name], [date], and measurable value placeholders with data drawn from baseline assessments. The SMART Target Writing Planner supports practitioners in drafting bespoke targets where needed, while the Target Review Record provides a structured format for documenting progress at each half-termly or termly review point. The Pupil Voice Sheet should be completed collaboratively with the child or young person, in line with the person-centered practice requirements of Section 19 and Section 43 of the Children and Families Act 2014. The quick-reference statutory guide and glossary make the resource particularly useful for staff who are newer to SEND legislation or who require a reliable compliance checkpoint during plan-writing.
